For legal, insurance, and work safety reasons, all forms must completed and signed before music tuition can proceed. Please print then fill-in everything you are able to, leaving the other unknown fields blank which we will complete with you at your first lesson. The important thing is that you sign the agreement forms.

If you have discussed your tuition price with us we will most probably suggest that you write it in the 1. Discount Period column next to the $ _____ otherwise just leave it blank and we will complete it with you on presentation of the forms.
Please Note: This 3 tiered payment schedule is used as an incentive for you to start paying by week 5 of term. If you do, you will save $40 on your 2. Standard Period fees. If you don't start paying until the 3. Late Fee Applies period then $80 will be added to your overall term fees. Additionally, if you start paying after the term has finished, a $22 administrative fee will be incurred.
So always remember, you get a reduced fee if you start paying before week 6 of term.

3. Student Music Tuition Registration Form - Fees Payment Method
Use this form to indicate your preferred payment method which can be via Credit Card, Direct Debit Bank Transfer, Cheque/Money Order, EFTPOS, Cash or a mix of any or all. You are welcome to pay off your fees over the term weekly, fortnightly, monthly and as long as you start paying by week 5 of term you will receive the Week 1-5 "Discount Period" rate.
